Tinnitus is an auditory condition that is often described by patients as a hissing, buzzing, roaring, clicking, ringing, whistling or whooshing sound either in one or both ears which is referred to as tinnitus aurium. If the sound is perceived from head then it is called tinnitus cranii. Tinnitus is characterized by the irritating sensation of hearing sound that has no external sound stimulus and it includes phantom noise which can differ from a low frequency roar to a high pitch sound and it can affect one or both ears. Rarely, the ringing sound can be very loud that it can distract affected people to focus or perceive the actual sound.
The occurrence of tinnitus varies in patients as it commonly subsides or may come and go and rarely be present all the time. The sound can be either a high pitch or low pitch and it can range between a single tone, multi-tonal, or noisy and without any tonal form. Tinnitus can begin abruptly and progress gradually. The symptoms of tinnitus can be constant, pulsing or intermittent. The signs and symptoms of tinnitus are based on the type of tinnitus. Tinnitus is broadly classified into two types: – objective and subjective.
The type of tinnitus must be determined before planning a treatment. The objective tinnitus occurs due to a physiological cause as the sound is perceived by the patient. An objective type of tinnitus indicates a presence of an underlying medical condition like muscle spasms, Temperomandibular joint problems which are associated with clicking sounds in the middle ear. The symptoms of pulsatile or vascular tinnitus include hearing sounds that are analogous to the patient’s own heartbeat as it is due to increased blood flow. Hence, an objective tinnitus is clinically audible to an observer either with the aid of stethoscope or by just listening on close proximity to the ear. The incidence of objective tinnitus is less than 5 percent and is often a result of vascular or muscular disorders. In many cases, the causative factor of objective tinnitus can be determined and the treatment options can be either medical or surgical.
The symptoms of subjective tinnitus are due to ear disorders and around more than 80 percent of the patients present with sensorineural hearing loss due to a nerve damage or damage to the ear ossicles. Clinically, subjective tinnitus is audible only to the patient and the severity of the condition varies based on every patient’s reaction. This form of tinnitus is more common and it is accounted in 95 percent of the tinnitus cases.
Some of the common signs and symptoms that are experienced by tinnitus patients include: – disturbed or difficult sleep, Inability to concentrate while hearing, depression, anxiety, irritation, confusion, increased stress levels. Long term patients encounter Musical hallucinations and hearing loss. These patients are more susceptible to increased sound sensitivity (hyperacusis). There may not be an apparent reason for these musical hallucinations and stress can often trigger musical hallucinations and worsen the condition. In severe cases, there can be hearing impairment.

Tinnitus treatment
Introduction: Tinnitus is an auditory condition that is characterized by ringing, swishing, or other form of noise that gets emanating inside the ear or head. In some cases, tinnitus indicates an underlying health condition. Tinnitus can originate from various parts of the ear such as the outer ear, the middle ear, the inner ear, and also due to some brain abnormalities. Normally, mild form of tinnitus or head noise is common in humans especially in sound proof areas as the normal body sounds are masked by the outside noise. Tinnitus occurs as a result of accumulation of ear wax or a foreign body implantation in the external ear as the background sounds will be masked which make us sensitive to our own head sounds. Tinnitus is generally caused by disease of the ear drum (tympanic membrane) or middle ear, fluid accumulation, and infection.
Treatment:
The physician suggests the treatment for tinnitus after performing a complete auditory evaluation to identify a causative factor that triggered the condition. The treatment modality is based on the cause, patient specific symptoms, type of tinnitus and options to effectively manage the condition. Read on to know the best effective treatment options to manage tinnitus.
Tinnitus medications
In majority of the cases, tinnitus has no specific treatment as it usually subsides on its own. However, rarely it may be associated with permanent visual impairment that has no cure. Ear specialists often recommend niacin drug to treat tinnitus without any claims of it in reducing tinnitus. Certain Anti-anxiety and Anti-depressant drugs are effective in treating tinnitus. The combination of certain drugs and vitamins has been found to work against tinnitus. Melatonin, IV Lidocaine, benzodiazepines, Tricyclic anti-depressants, Zinc supplementation, Acamprosate, Etidronate or sodium fluoride, Lignocaine or anticonvulsants, Carbamazepine, Sertraline, and Bupropion.
Lifestyle Changes
Certain lifestyle modifications can work in reducing the irritation and intensity associated with tinnitus. Hence, a healthy diet can play an important role in addressing the cause. The intensity of the sound gets louder with increased consumption of caffeine (coffee, tea). Smoking is another cause that increases the perception of noise and discontinuing the habit can help to manage tinnitus. Additionally, avoiding caffeine including drugs like aspirin can decrease the annoyance caused by tinnitus.
Tinnitus masking devices and Ear plugs are advised for patients with tinnitus to minimize the volume and stabilize the sound. It is an effective aid to minimize the high decibel frequency and in preventing further damage to the inner ear that impairs hearing. White noise machines may not cure tinnitus however these can create a relaxing, noise-free atmosphere which can induce sleep at night. It is advised to effectively manage the stress, anxiety and depression to rule out the psychological stimulus of tinnitus as these symptoms can worsen the condition. Practicing relaxation techniques and meditation can work towards relieving stress.
The treatment for objective tinnitus includes Gamma knife radiosurgery, Shielding of cochlea by teflon implant, Botulinum toxin, Cleaning ear canal in the case of earwax plug, and use of neurostimulator can effectively reduce the symptoms associated with tinnitus.

Tinnitus
Introduction
Tinnitus is derived from a Latin word “tinnitus” which means “ringing”. Tinnitus is a symptom within the human ear that is characterized by the perception of sound in the absence of an external sound stimulus. It occurs by a wide range of causes such as ear infection, due to implantation of foreign objects inside the ear, nasal congestion, wax accumulation, high volume ear headphones. However, commonly tinnitus is a cause of noise induced hearing loss. It is a result of sudden withdrawal of long term medications like benzodiazepine. Tinnitus can be either acquired due to aging process associated with natural hearing impairment, side effect of some drugs or it can be congenital leading to genetic hearing loss.
Causes
Tinnitus more frequently affects white individuals and its prevalence in the U.S. is twice higher in the South compared to Northeast. The causes of tinnitus can be divided into objective and subjective based on its nature of occurrence:-
1. Objective tinnitus
Objective tinnitus can be perceived clinically as an actual sound originating from an affected ear. It can be due to the spasm of muscles that cause clicking sound around the middle ear. Objective tinnitus occurs due to twitching of the ear or throat muscles, and TMJ problems. Increase in the blood turbulence or alterations in the blood flow to the ear due to atherosclerosis or venous hum, carotid artery aneurysm, vasculitis carotid artery dissection, giant cell arteritis. It is rarely caused by benign intracranial hypertension, pregnancy, anemia, hyperactive thyroid gland and tumors involving ear.
2. Subjective tinnitus
Subjective tinnitus is commonly caused by otologic conditions that lead to hearing impairment. It is usually due to noise-induced hearing loss as a result of exposure to loud sound. Certain medications are Ototoxic and can lead to hearing loss despite administering these drugs at dosages that are not ototoxic. Drugs like aspirin, quinidine, and around more than 260 medications have been claimed to induce tinnitus as a side effect. Tinnitus can persist for several months as benzodiazepine withdrawal syndrome. Some of the causes of subjective tinnitus include ear infection, over-accumulation of ear wax (Cerumen), ear trauma affecting both ears, and exposure to loud noise. Medications like aminoglycoside antibiotics, NSAIDS, ciprofloxacin, erythromycin, chloramphenicol, tobramycin, vancomycin, tetracycline, doxycycline, chemotherapeutic drugs, anti-viral drugs, loop diuretics and anti-depressants can cause severe ototoxicity. Brain tumors like acoustic neuroma rarely cause tinnitus.
Tinnitus is caused by Conductive hearing loss, acoustic shock, external ear infection, middle ear effusion and superior canal dehiscence. Other pathological causes include loss of sensori-neural hearing due to presbycusis (age-associated hearing loss), acoustic neuroma, Meniere’s disease and mercury or lead poisoning.
Types
Tinnitus occurs in two different types.
Pulsatile tinnitus: It occurs due to the sound formation by muscle movements around the ear, ear canal alterations, cervical or facial vascular conditions. This condition presents with hearing sounds analogous to contractions of muscles, heart beat or pulse.
Non pulsatile tinnitus: It is a condition caused by the nerve defect that is associated with hearing. It affects one or both ears and this type of tinnitus is perceived to be emanating internally from the head.
